ABSTRACT

Objective To compare postductal heart rate and saturation (SpO2) measurements from the wireless PO device obtained by iVital+against measurements by the standard Masimo (SET technology) monitor in the monitoring of neonates. Methods Pulse oximetry reading of newborns were assessed in terms of heart rate and saturations with two PO simultaneously attached to postductal site and data comparison was done. Results Out of the 1000 cumulative recordings, the mean diference between HR obtained from both PO was 0.415 and level of agreement was 2.3 beats per minute. For SpO2 mean diference between devices was 1.21 and level of agreement was 1.5%. There was very little diference between SpO2 measurements when the Masimo SpO2 was?70%. Conclusion As this pulse oximeter is small, portable and accuracy is as comparable to Masimo, this provides a good solution for efcaciously monitoring neonates. It can also be used in the monitoring of children with suspected or afected with COVID-19 in hospital and ICU settings as also in the quarantine facilities. This reduces the need for constant presence of medical and nursing personnel.

ABSTRACT Objective: To understand the perception of adults with obesity about remote monitoring at the beginning of the pandemic of COVID-19. Methods: A descriptive-exploratory, qualitative study with participants in remote monitoring of a multi-professional treatment program in southern Brazil. Data was collected through digital platforms with interviews in written or audio-recorded records. The results were submitted to content analysis, thematic modality. Results: 26 women participated, emerging the thematic category "Difficulties and potentialities in adherence to lifestyle changes during the pandemic. Among the difficulties are the change of routine and increased anxiety; and, among the potentialities are the multi-professional accompaniment, decreased tension, and weight control. Final considerations: The participants' perception showed considerable weaknesses in adhering to the program, caused by social distance. Studies with remote interventions are essential to improve the quality of this type of health care.


RESUMEN Objetivo: Comprender la percepción de adultos con obesidad bajo telemonitorización en el inicio de la pandemia de COVID-19. Métodos: Estudio descriptivo-exploratorio, cualitativo, con participantes en acompañamiento remoto de un programa de tratamiento multiprofesional de obesidad en el Sur brasileño. ABSTRACT

Due to the COVID-19 pandemic, the face-to-face interventions of the Academia da Cidade of Recife (PAC-REC) were suspended and a remote activity plan was elaborated. The aim of this study was to describe the implementation and results of remote PAC-REC activities during the COV-ID-19 pandemic. A quantitative and qualitative analysis was performed based on the PAC-REC online reports and the virtual platforms referring to the remote activities carried out, from April to July 2020. For the implementation of the remote activities, integrated and systematic planning and monitoring meetings among the professionals and the program management were made. The following material was produced: 14 lives treams of bodily practices and physical activities, 10 interviews, 70 videos with different themes, 85 infographics, among others specified by pole. It is concluded that the performance of remote activities of PAC-REC reached its objectives and some should be maintained after the return of face-to-face activities


Devido à pandemia da COVID-19, as intervenções presenciais do Programa Academia da Cidade do Recife (PAC-REC) foram suspensas e um plano de atividades remotas foi elaborado. O objetivo deste estudo foi descrever a implantação e os resultados das atividades remotas do PAC-REC durante a pandemia da COVID-19. Foi realizada uma análise quantitativa e qualitativa a partir dos relatórios online do PA-C-REC e das plataformas virtuais referentes às atividades remotas realizadas, dos meses de abril a julho de 2020. Para implantação das atividades remotas foram realizadas reuniões integradas e sistemáticas de planejamento e monitoramento entre profissionais e gestão do Programa. Foram desenvolvidas 14 lives de práticas corporais e atividade física, 10 entrevistas, produzidos e divulgados 70 vídeos com temas diversos, 85 infográficos, entre outras específicas por polo. Conclui-se que a realização das atividades remotas do PA-C-REC alcançou os seus objetivos e algumas deverão ser mantidas após o retorno das atividades presenciais

ABSTRACT

Clinical trial management system is independently developed by our hospital, which basically realized the whole process management and data collection of clinical trials. Based on the platform, the functional architecture of data remote monitoring and auditing was established. By desensitizing and encrypting of data, the project and subject hologram were visualized to facilitate to review of data. The data remote monitoring and auditing cloud platform adopts the B/S architecture pattern. Users register to apply for an account through the cloud platform, and access to the account via HTTPS security protocol. The authorized users were able to view the relevant items online to ensure the secure data transmission and easy operating. The electronic management of data is the direction of future efforts. By compliance with laws and regulations, the remote monitoring/auditing can be realized, and the data security and personal privacy can be ensured with the application of information technology. In this paper, the feasibility of remote monitoring/auditing mode is explored, specific technical schemes and system functions are suggested, and the realization scenarios are conceived in case of major public health emergencies.

ABSTRACT

Objective: To carry out effective management for the cold-chain temperature of in vitro diagnostic reagent so as to provide safe and credible inspection basis for clinical diagnosis and treatment. Methods: The remote control and alarm platform of cold storage and refrigerator were applied to achieve acceptance for temperature control of in vitro diagnostic reagent, and achieve acceptance for temperature and achieve differentiation management for unqualified product. These methods could ensure controllability of temperature for the reagent with requirements of cold-chain. Results: Through the management of cold-chain of in vitro diagnostic reagents, hospital has achieved effective supervising for them, and hospital has concrete record for these reagents in the entire medical process, and all of them were traceability. Therefore, it provide effective guarantee for clinical safety. Conclusion: The support of informatization technique and implementation of management system of hospital can ensure the cold-chain management of entire process is not out of control, and enhance the stability and accuracy of clinical test results and effective guarantee the safety of diagnosis and treatment for hospital.

Objective To investigate the application of remote monitoring systems for cardiac implantable electronic devices (CIED) and its success rate of data transmission in elderly patients.Methods A total of 97 elderly patients who had previously undergone procedures for pacemaker implantation,implantable cardioverter defibrillators (ICD) or cardiac resynchronization therapy (CRT) with remote monitoring capabilities between January 2013 and October 2016 at our hospital were enrolled.We evaluated the effect of the remote monitoring systems for data transmission,compared the outpatient follow-up rates between the groups one year after implantation,and conducted a telephone survey of patients.Results A total of 97 elderly patients,including 70 with pacemakers,20 with ICD and 7 with CRT,were enrolled in this study.Participants had a mean age of (78.2-±-6.4) years and 64 were male (66.0%).The Home Monitoring system was used for 85 cases and the Merlin.net remote monitoring system was used for 12 cases.Overall,95 patients (97.9 %) completed the remote monitoring procedure,68 patients (70.1%) completed the outpatient follow-up,and 90 patients (92.8%) completed the telephone survey during a one-year follow-up period.The satisfaction rate for the remote monitoring systems was 94.4% (85 patients),and 80 patients (90.0%%) expressed a willingness to continue to use the remote monitoring system if a replacement was needed.Conclusions For elderly patients with CIED,the remote monitoring systems can improve the follow-up rate with a high degree of satisfaction.

Objective:To investigate the clinical accuracy of cardiac remote monitoring, echocardiography (ECHO) and dynamic electrocardiography (DCG) in the diagnosis for silent myocardial ischemia (SMI) of coronary heart disease (CHD).Methods:63 patients with CHD were enrolled in this retrospective analysis and coronary angiography (CGA) was used as gold standard to analyze and judge the sensitivity, accuracy, specificity, positive predictive value, negative predictive value, Youden index and Kappa value of cardiac remote monitoring technique, ECHO and DCG in the diagnosis for SMI.Results: As the results of gold standard (CGA), 41 cases of 63 patients with CHD were diagnosed as SMI, the diagnose accordance rates of cardiac remote monitoring technique, ECHO and DCG, respectively, were 92.68%, 53.66% and 90.24%. And the sensitivity and specificity of the three methods for SMI, respectively, were 92.68% and 77.27%, 53.66% and 90.24%, and 86.36% and 22.73%. The Youden Indexes of the three methods were 0.700, 0.236 and 0.766, respectively. As above results, the Youden Indexes of cardiac remote monitoring technique and DCG were obviously higher than that of ECHO, and their diagnosis for SMI has excellent accordance with CGA. Conclusion:The technique of cardiac remote monitoring can be used as an effective supplementary means of DCG, and it has great clinical significance in the early diagnosis for SMI.

ABSTRACT

<p><b>INTRODUCTION</b>Remote monitoring of cardiac implantable electronic devices (CIED) has been shown to improve patient safety and reduce in-office visits. We report our experience with remote monitoring via the Medtronic CareLink(®) network.</p><p><b>METHODS</b>Patients were followed up for six months with scheduled monthly remote monitoring transmissions in addition to routine in-office checks. The efficacy of remote monitoring was evaluated by recording compliance to transmissions, number of device alerts requiring intervention and time from transmission to review. Questionnaires were administered to evaluate the experiences of patients, physicians and medical technicians.</p><p><b>RESULTS</b>A total of 57 patients were enrolled; 16 (28.1%) had permanent pacemakers, 34 (59.6%) had implantable cardioverter defibrillators and 7 (12.3%) had cardiac resynchronisation therapy defibrillators. Overall, of 334 remote transmissions scheduled, 73.7% were on time, 14.5% were overdue and 11.8% were missed. 84.6% of wireless transmissions were on time, compared to 53.8% of non-wireless transmissions. Among all transmissions, 4.4% contained alerts for which physicians were informed and only 1.8% required intervention. 98.6% of remote transmissions were reviewed by the second working day. 73.2% of patients preferred remote monitoring. Physicians agreed that remote transmissions provided information equivalent to in-office checks 97.1% of the time. 77.8% of medical technicians felt that remote monitoring would help the hospital improve patient management. No adverse events were reported.</p><p><b>CONCLUSION</b>Remote monitoring of CIED is safe and feasible. It has possible benefits to patient safety through earlier detection of arrhythmias or device malfunction, permitting earlier intervention. Wireless remote monitoring, in particular, may improve compliance to device monitoring. Patients may prefer remote monitoring due to possible improvements in quality of life.</p>

El mayor cumplimiento de las guías de práctica clínica y los avances tecnológicos implementados en los dispositivos cardiacos, han demostrado mejorar la calidad de vida y supervivencia de los pacientes portadores de ellos, quienes requieren de seguimiento periódico como parte del proceso terapéutico, pero la complejidad de su control, las frecuentes alertas de seguridad de generadores y electrodos, y el aumento del número de portadores, sobrecargan las consultas, obligando a prolongar el intervalo entre revisiones. La monitorización remota es un sistema que proporciona a distancia y en tiempo real, información completa sobre el estado y funcionamiento de los dispositivos cardiacos implantables, permite optimizar el control del paciente, evitar desplazamientos innecesarios y reducir los costos sanitarios. Objetivos: Evaluar la práctica clínica actual sobre monitorización, las funciones de enfermería, revisar y rediseñar el protocolo de seguimiento como área de mejora. Metodología: Se realizó una búsqueda bibliográfica utilizando los descriptores MeSH: desfibrilador implantable, marcapasos artificial, monitorización remota y atención de enfermería, en las bases de datos: Medline, Embase, Cochrane, Cuiden y CINAHL acotada a los trabajos publicados en los últimos seis años. La búsqueda se ha basado en la revisión de nuevos modelos organizativos de monitorización remota, se analizaron los resultados y se hizo una evaluación crítica de las intervenciones para actualizar el protocolo de la consulta de enfermería. Resultados: Las directrices actuales para disminuir la variabilidad asistencial definen los objetivos, frecuencia, contenido del seguimiento remoto y funciones de los profesionales implicados. Con base en la evidencia y la experiencia acumulada se ha actualizado el protocolo de atención a estos pacientes. Conclusiones: La monitorización remota requiere profesionales de enfermería expertos en estimulación cardiaca


A better compliance with clinical practice guidelines and technological advances implemented in cardiac devices improve the quality of life and survival of patients carrying them. These patients require periodic monitoring as a part of the therapeutic process, but the complexity of their control, the frequent security alerts from generators and electrodes, the increase in the number of carriers, fill up the consulting rooms, and consequently the interval between checkups needs to be longer. Remote monitoring it is a system that provides remote, real-time, comprehensive information about the condition and operation of implantable cardiac devices, and allows patient’s management to be optimized, unnecessary displacements to be avoided and healthcare costs to be reduced. Objectives: To evaluate the current clinical practice on monitoring and the nursing roles, and review and redesign our monitoring protocol as an area of improvement. Methodology: A bibliographical search was conducted using the MeSH descriptors: implantable defibrillator, artificial pacemakers, remote monitoring and nursing care, in the databases: Medline, Embase, Cochrane, CINAHL take care and bounded to the papers published in the last six years. The search is based on the review of new organizational models for remote monitoring, the results thereof were analyzed and a critical evaluation of our interventions was made in order to update our nursing consultation protocol. Results: The current guidelines to reduce variability of care define the objectives, frequency, content and functions of the remote monitoring of the professionals involved. Based on the evidence and our experience, has been updated the protocol of care for these patients. Conclusions: Remote monitoring requires skilled nursing professionals in cardiac stimulation

Combined with RFID tags, readers etc., ICU patients are effectively identified and identifying information transmission and computer management are complied. And then a remote monitoring system for ICU is achieved.Results: The system realizes the ICU remote monitoring and effective prevention of nosocomial infections, and meets the families of patients remote access needs; the system can integrate with HIS/CIS seamlessly to enhance the ICU clinical services integrated computerized management further.Conclusion: The remote monitoring system for ICU by technology of RFID-based internet of things, with strong practicability, can be widely applied.

Este trabalho apresenta um protótipo de caixa de condicionamento operante para ratos - batizada de Caixa de Atividades Diárias Para Ratos - que permite o registro, 24h por dia, de múltiplos operantes. O sujeito experimental pode ser remotamente monitorado (visto e ouvido em tempo real) por meio de uma webcam localizada na frente da caixa. Além disso, todas as funções do equipamento podem ser controladas remotamente via internet. Trata-se da primeira caixa experimental a reunir todas estas funções, ao menos no Brasil. This work presents a prototype of an operant conditioning box for mice - named Daily Activities Box for Rats -, in which multiple operants can be recorded 24 h per day. The experimental subject can be monitored remotely (seen and heard in real time) by means of a webcam located in front of the box. Furthermore, all the equipment functions can be controlled remotely via internet. This is the first experimental box to gather all these functions, at least in Brazil. Objective To investigate the reliability of electrocardiographic (ECG) signal for the accurate assessment of myocardial ischemia in order to evaluate the clinical value of remote real-time ECG monitoring system based on GPRS in patients with acute ST-elevation myocardial infarction (STEMI).Methods A total of 60 STEMI patients admitted between April 2008 and December 2010 were enrolled.All subjects were given the remote real-time ECG monitoring and routine 12 leads ECG monitoring at the same time.They were divided into remote ECG group and the 12 leads ECG group.The remote real-time ECG monitoring collects electrocardiosignal to imitate V1,V3,V5 lead and Ⅰ lead.P wave duration,PR interval,duration of time limit of QRS wave and T wave,QT interval,and the P wave amplitude,QRS wave amplitude,R-(Q + S),T wave amplitude were measured,and the detectability rate of arrhythmia and the definited diagnosis rate of ST segment elevation in accordance with clinical manifestion were compared in each group with different parameters.The data were analyzed by t test,rank sum test,Pearson correlation analysis,Spearman's rank correlation and the chi-square test.Results In STEMI patients,there were no statistical differences in time limits and amplitude of waves on ECG between the two groups (P > 0.05),and the correlations between parameters of two groups were found to be close (P < 0.01).There was no difference in the detectability rate of cardiac arrhythmia between two groups (P > 0.05),and no difference in the rate of correct diagnosis of elevated ST segment between two groups (P > 0.05),except V1 lead (P < 0.05).Conclusions The sensitivity of the change in ST segment of the two groups is similar,and the remote real-time ECG monitoring can help determine the location of myocardial ischemia.

ABSTRACT

A portable, Internet-based EEG/Auditory Evoked Potential (AEP) monitoring system was developed for remote electrophysiological studies during sleep. The system records EEG/AEP simultaneously at the subject's home for increased comfort and flexibility. The system provides simultaneous recording and remote viewing of EEG, EMG and EOG waves and allows on-line averaging of auditory evoked potentials. The design allows the recording of all major AEP components (brainstem, middle and late latency EPs) and constituent single sweep EEG epochs. Auditory stimulation is remotely controlled from any internet-connected PC. The developed platform consists of a portable server computer for data acquisition and secured data transmission to the Internet, a web server for temporary storage, and a remote viewing station. During operation, signals are acquired during sleep and transmitted to the secured web server for controlled access viewing. The remote viewer station allows the user to continuously visualize the auditory evoked potentials, modify auditory stimuli at any time and adjust system parameters for its online computation. The developed internet-based EEG/EP system has been tested in a laboratory environment. Preliminary results demonstrate feasibility of remote real-time acquisition and viewing of EEG and EPs and future potential for home monitoring and telemedicine applications.

In this paper an expert system for remote fault diagnosis in the ship lift was developed by analysis of the fault tree and combination with VPN. The fault tree was constructed based on the operation condition of the ship lift. The diagnosis model was constructed by hierarchical classification of the fault tree structure, and the inference mechanism was given. Logical structure of the fault diagnosis in the ship lift was proposed. The implementation of the expert system for remote fault diagnosis in the ship lift was discussed, and the expert system developed was realized on the VPN virtual network. The system was applied to the Gaobaozhou ship lift project, and it ran successfully.

ABSTRACT

OBJECTIVE: Telemedicine uses common technologies that provide a conduit for information exchange between physicians nurses and patients. In addition to patient records, medical professionals can obtain vital signs and other reference data through telemedicine applications. also, recent advancement in RF technology and wireless communications has enabled the development of noble networks. Mobile telemedicine involves more than just communicating via mobile phone of PDA that sends and receives medical data on Portable Internet. METHODS: This paper describes the design of a mobile telemedicine PDA prototype based on Embedded Linux that can be used to monitor the medical information using a single board computer(SBC). RESULTS: The device hardware architecture consist of signal conditioning circuit, single chip micro-controller, LCD display and wireless network interface card(W-NIC). Embedded Linux was ported as its operating system and application software was developed QT/embedded 2.3.7. CONCLUSION: This system would be very useful for patients with chronic illness, especially in an emergent environment, and for containing medical cost in the aged society.
